Objectives
After completing the course, the student is able to
• prepare a comprehensive client anamnesis and conduct client examination with appropriate tools, methods, and techniques
• order and/or conduct additional examinations and interpret their results to determine the need for treatment
• draw evidence-based conclusions about his/her client's health independently in his/her own area of expertise, plan care/treatment and send the client to the treatment (referral) if necessary
• take responsibility for the care of clients with non-communicable diseases in the different phases of their treatment paths independently or by consulting, if necessary, the multiprofessional team
• guide /inform the client about the benefits of medicines, possible side effects, combined effects and importance to follow the instructions as part of the health self-management
• support the client´s rehabilitation
• monitor and evaluate the health, the effects of care and health self-management of clients with long term conditions
• assess and develop the client and drug safety at the work unit level
Content
• Comprehensive assessment and clinical examination of the client's health
• Medical and non-medical treatment of the client from the point of view of nursing
• Supporting rehabilitation and support methods for rehabilitation
• Reception of an advanced clinical practitioner
• Development of client and drug safety
• Health and care plan as part of the chronic care model
