Supervised Clinical Training: Nursing Care of a Child or an Adolescent who is Suddenly Ill and Needs Intensive Care (9 cr)

Objectives (course unit)

The student
- works responsibly, safely and ethically in paediatric nursing care
- utilises and applies evidence-based knowledge in nursing care and working life for children and families
- is able to plan, implement, evaluate and document a child’s and adolescent’s nursing care in a multidisciplinary way with the child and the family
- knows the central methods for paediatric in nursing care
- is able to implement pharmacotherapy and fluid therapy in paediatric nursing together with the mentor nurse and knows the drug treatment practices of the particular department
- is able to guide and support children and families of different ages at various phases of the care process, promoting health and functional performance
- is able to plan, explain and manage his/her own actions and understands the importance of those actions
- acts collegially as a representative of his/her profession in multi-professional working groups
- is able to choose economically sustainable methods
- is able to evaluate the development of his/her own profession in the care of a ill child and identifies in himself/herself the need for continuous development

Supervised clinical training in paediatric nursing, 6 weeks

Prerequisites (course unit)

At the beginning of the elective vocational studies, the student must have completed 135 credits. These should include the theoretical studies and orientation to practical training of the area which the student has chosen for the elective studies, as well as courses in anatomy and physiology. The required area is the course of Paediatric Nursing Care

Assessment criteria, pass/fail (course unit)

Pass
The student
- is able to independently apply the knowledge and skills of nursing care for children and adolescents
- is able to plan, implement, evaluate and document the nursing care of his/her own paediatric patient in cooperation with the supervising nurse, the child and the family
- knows the central care procedures in paediatric nursing
- is able to safely carry out pharmaceutical and liquid therapy for children together with the supervising nurse and knows the medication care practices of the department
- knows the role of the treatment unit for the child and family as part of the care and service chain
- meets with the child, family and work community in a meaningful and respectful manner and is able to interact professionally with the child, family and members of the work community
- is able to guide and support children of different ages and their families at different phases of the care process with the supervising nurse
- is able to act as a representative of his/her own profession in a multi-professional work community
- acts responsibly, recognising the limits of his/her skills and is able to act safely with the child and the family
- follows the confidentiality guidelines and the regulations of patient data protection
- adheres to the agreed rules of clinical practice in his/her own actions
- reflects and evaluates his/her own activities realistically, and is able to give and receive feedback

Fail
The student
- has a clear lack of theoretical knowledge and skills of nursing care of children and adolescents, even after the tutoring discussions;
- is not able to plan, implement, evaluate and document the care of his/her own named child; performance is incomplete and does not contribute to the well-being of the child and family
-does not manage the key paediatric methods
- makes mistakes in pharmacotherapy repeatedly despite guidance
- does not perceive the care path of his/her own child and family
- treats the child, family and work community inappropriately and disrespectfully and is unable to interact professionally with the child, family or work community members
- does not recognise the need for a child’s family guidance and support and does not show willingness to participate in them
- is unable to act as a representative of his/her profession in a multi-professional work community
- does not recognise the limits of his/her competence, endangering the child's safety
- violates confidentiality or patient privacy
- is not able to follow the agreed rules for supervised practice (e.g., does not take responsibility for his/her own learning, learning goals are missing, does not follow the working hours, does not report absences, the hours of practice are not met)
- evaluates his/her own skills unrealistically and is unable to receive feedback

Exam schedules

Mahdolliset poissaolot korvataan opettajan ja harjoitteluyksikön kanssa sovitusti, pääsääntöisesti jakson aikana, toissijaisesti jaksoa pidentämällä tai seuraavalla jaksolla.

Lisäpäivistä täytyy tehdä erillinen sopimus harjoitteluyksikön kanssa.

Harjoittelu on mahdollista myös opinnollistaa/ahotoida, jonka mahdollisuudesta keskusteltava etukäteen vastuuopettajan kanssa.

Student workload

6 viikkoa ( 9 op x 27 h) eli harjoittelujakson kokonaistuntimäärä on 243 h, joista osastolla tehdään 227 h. Juhlapyhän osuessa arkipäivälle voi osastotunneista vähentää 7h/päivä.

20shlas2: Taitokoe harjoittelun aikana ja hoitosuunnitelma yhdestä lapsesta ja perheestä, jonka hoitoon on osallistunut. Tarkemmat erilliset ohjeet ryhmän moodlessa.

21shlas1: Lapsen valmistaminen hoito- tai toimenpiteeseen ja kirjallinen tehtävä harjoittelun aikana. Tarkemmat erilliset ohjeet ryhmän moodlessa.

Arkipyhien osuessa harjoittelujaksoon voi harjoittelun tunneista 7h/päivä.

Opinnollistaminen:
lasten hoitotyön vaihtoehtoisten ammattiopintojen ohjatun harjoittelun voi tehdä opinnollistaen lasten somaattisella osastolla/hoitoyksikössä tai lastensuojelun hoitoyksikössä, mikäli harjoittelun tavoitteet täyttyvät. Yhden harjoittelun voi opinnollistaa edellä mainituissa harjoitteluyksiköissä.
Ehtona opinnollistamiseen on:
-ohjatun harjoittelun opintojakson tavoitteet täyttyvät.
-vähintään joko kevään tai syksyn lasten ja nuorten vaihtoehtoisten ammattiopintojen teoriaopinnot on suoritettuna

Mikäli opiskelijalla ei ole sijaisuudessa lääkelupia, joissa opiskelijalla on suonensisäisiin lääkkeisiin/nestehoitoon lupa, opiskelija itse sopii työpaikan kanssa siitä, että hän osallistuu vähintään yhtenä päivänä (U-päivä) osaston lääkehoitoon (sh:n, osastofarmaseutin tms. kanssa).

Ahotointi:
-yhden harjoittelun tai harjoittelun osan voi ahoitoida sairaanhoitajan yhtäjaksoisella palkallisella työllä
-vähintään joko kevään tai syksyn lasten ja nuorten vaihtoehtoisten ammattiopintojen teoriaopinnot on suoritettuna

Esitietovaatimukset
Suoritettuna hoitotyön alku- ja keskivaiheen opinnoista, vähintään 135 op sisältäen Lasten, nuorten hoitotyön –opintokokonaisuuden, ja Lasten ja nuorten hoitotyön orientoivan harjoittelun.

Osallistuminen 7K00EY22-3002 Äkillisesti sairaan ja tehohoitoa vaativan lapsen ja nuoren hoitotyö - kokonaisuuteen.

Ohjeita harjoitteluun:
- Täytä lääkehoidon passia jakson aikana.
- Laadi harjoittelujakson ensimmäisellä viikolla tavoitteet arviointilomakkeelle.
- Laadi harjoitteluohjaajien työvuoroja mukailevat työvuorot harjoittelujaksolle kirjattuna työvuorolomakkeelle. Kirjaa myös ohjaajasi nimikirjaimet kunkin työvuoron kohdalle.
- Lähetä ohjaavalle opettajalle työvuoroehdotukset ja alustavat tavoitteet arviointilomakkeelle kirjattuna, laita ko. sähköposti samalla kopiona myös harjoitteluohjaajille ensimmäisen viikon aikana.
- Ohjaava opettaja antaa sähköpostilla palautetta tavoitteista opiskelijalle sekä ohjaajille ja sopii väli- ja loppuarviointiajankohdat opiskelijan kanssa.
- Väliarviointiaika (3 viikolle)sovitaan heti jakson alussa. Väliarviointi käydään opiskelijan, ohjaajien ja opettajan kesken. Opiskelija valmistautuu väliarviointiin etukäteen kirjaamalla itselleen oppimansa asiat ja kehittämishaasteet.
- Harjoittelun puolessa välissä on mahdollisuus tulla reflektiokeskusteluun koululla, etänä tai erikseen sovitusti harjoitteluyksikössä. Reflektiokeskustelu sisältyy harjoittelutunteihin.
- Loppuarviointikeskustelu käydään opiskelijan, ohjaajan ja opettajan kesken. Mikäli kaikille osapuolille sopivaa ajankohtaa ei löydy, sovitaan arviointitavasta erikseen. Opiskelija tekee kirjallisen itsearvioinnin arviointilomakkeelle ja lähettää sen sähköpostilla opettajalle ja ohjaajille kaksi päivää ennen loppuarviointia.
