Objectives (course unit)

The student
- performs the normal vaginal birth independently
- is able to plan and independently do decisions regarding the care during birth
- is able to guide the family during birth
- recognises the abnormalities in labour progression and manages them
- recognises the signs of foetal distress and acts properly
- knows the differences between low and high risk births, understands the special features of them and implements the knowledge of midwifery properly during the both types of births
- is able to perform the breech delivery if the medical professional is not available
- is able to work as the member of multi-professional team in the situations requiring quick decisions
- knows how to assist and perform the abnormal births
- is able to do the preparations for operations during birth
- is able resuscitate and to support the transition of newborn baby
- is able to support the physiology of birth and the development of positive birth experience
- develops the expertise of midwifery during childbirth

Content (course unit)

- midwifery during normal and abnormal birth
- care plan and decision making under supervision
- importance of birth and birth experience on sexual and reproductive health of women and their families
- assistance during abnormal birth and preparation for caesarean section
- customer oriented care, observation and support of early interaction of women given birth by caesarean section
- assessment of labour progression and its abnormalities
- assessment of maternal wellbeing during labour
- management of pain relief, fluid therapy and medication
- clinical and technology based observation of foetal wellbeing
- perineal support, episiotomy cutting and suturing, suturing of perineal tears
- postnatal maternal observation
- care and wellbeing observation of a newborn baby
- meaning of interventions performed during childbirth
- comprehensive understanding of midwifery during childbirth
- 6 weeks supervised clinical training in delivery rooms

Prerequisites (course unit)

Supervised Clinical Training I and II completed.
Two of the three supervised clinical trainings of Midwifery during Childbirth should be done at the university level hospital.

Assessment criteria, pass/fail (course unit)

Passed
The student
- is able to implement the holistic midwifery care safely and responsibly
- is able to recognize/ prevent the changes/complications in the wellbeing of woman in labour and foetus / woman at postnatal phase and newborn baby, and is working as the active member of multi-professional team
- is able to use and justify the use of observation methods during birth properly
- has the knowledge of midwifery care during childbirth and the interfaces between the disciplines related to that, recognises gap of knowledge and seeks to improve evidence based knowledge of midwifery
- is able to perform the normal and abnormal birth safely and in the customer oriented way
- meets the woman in labour and her support person and work group members professionally
- guides the woman in labour and her family in the customer oriented way
- follows the confidentiality and data protection guidelines
- follows in his / her actions the agreed rules of the supervised practical training
- evaluates her/his own competence in midwifery during childbirth and development of expertise in midwifery realistically and is able to give and receive feedback

Failed
The student
- is not able to plan, implement and assess the care of women in labour, there are wide shortcomings in knowledge of midwifery care during childbirth
- is not able to observe responsibly maternal and foetal wellbeing during labour and at the postnatal phase with the adequate methods, is neither able to anticipate or recognise changes in these and does not report these ahead
- meets the pregnant woman, family and work group members unprofessionally
- does not recognise the boundaries of his/her expertise, is endangering patient safety
- violates confidentiality or patient privacy
- fails to follow the agreed rules for supervised practice
- evaluates her/his own competence of midwifery care during childbirth unrealistically and is unable to receive feedback

Assessment methods and criteria

Jakson tavoitteet on kirjattuna ja ne on hyväksytty ohjaavan kätilön ja opettajan toimesta ensimmäisen harjoitteluviikon aikana. Työvuorosuunitelma on toimitettu ohjaavalle opettajalle ensimmäisen viikon aikana tai heti kun se on ollut opiskelijan saatavilla.
Palaute- ja arviointikeskustelut ohjaavan kätilön ja opettajan kanssa toteutettuna sovitusti.
Työvuorotaulukko ja arviointikaavake on oltava asianmukaisesti täytettynä ja harjoittelun jälkeen ohjeiden mukaan palautettuna. Lääkehoitopassia on täytetty harjoittelun edetessä. EU-direktiivien määrälliset tavoitteet tulee saavuttaa harjoittelun aikana.

Student workload

Synnytyssaliharjoittelussa tunnit tehdään täysimääräisinä EU-direktiivien määrällisten tapausten saavuttamiseksi. Jakson kokonaistuntimäärä määräytyy opintopisteiden mukaan: 1op = 27h.
(6op vaatii harjoittelua 4 viikkoa, tunteina 162h, 9op 6 viikkoa eli 243h.
Harjoittelun tunneista saa vähentää tavoitteiden ja itsearvioinnin kirjoittamiseen kuluvan ajan, eli n. 8 h.
Opiskelijan harjoitteluyksikössä tehtävä tuntimäärä on 38h 15 min/viikko. 4 viikon (6 op) harjoittelujaksolla, kokonaistuntimäärä on162 h,
joten opiskelija tekee harjoittelupaikassa 153 h. 6 viikon (9 op) harjoittelujaksolla, joissa kokonaistuntimäärä on 243h, opiskelija tekee 230 h.
