Supervised Clinical Training 5: Postnatal Nursing and Paediatric Nursing (6 cr)

Objectives (course unit)

The student
- is able to assess the need for nursing care for a child and a family at different ages; to plan, implement and evaluate care; and to gain experience in the entire care pathway, including discharge and the planning for coping/ managing of home care and continuity of care
- is able to apply evidence-based knowledge in childcare and to work ethically and responsibly, taking into account the cultural background of the family
- is able to observe the child and knows basic methods of nursing implementations
- can face challenges professionally and interact with the child and the family
- is able to identify issues in child growth and development, and to guide and support patient and family in child growth and development, health promotion, support and maintenance of resources
- is able to conduct pediatric pharmacotherapy and fluid therapy safely, under supervision
- is able to observe the child and knows the basic skills of nursing actions
- is able to take appropriate care in emergency childbirth and act effectively in case of resuscitation
- is able to monitor the newborn and identify common disorders
- is able to guide parents in a newborn's basic care and in breastfeeding
- can observe and support parents in early interaction
- is able to observe the postpartum recovery and well-being of the mother and to recognize disorders
- is able to evaluate his/her own skills and development

Content (course unit)

- holistic nursing care of the patient
- supervised clinical training two weeks postnatal nursing care and two weeks paediatric nursing care in the neonatal intensive care unit

Prerequisites (course unit)

Supervised Clinical Trainings 1, 2. 3 and 4 completed.
After the student has participated in both theoretical nursing studies and the orientation to clinical training courses involving his / her chosen area of specialization, he/she can go to the supervised practical training.

The student
- treats children of different ages and the whole family, as wells as members of the work community, in a meaningful and respectful way and is capable of multidisciplinary cooperation
- is able to interact professionally with children of different ages and the whole family
- perceives the care path for children of different ages, a woman who has given birth and the whole family
- is able to plan, implement and evaluate care for children of different ages and for the whole family in a client-oriented way
- is able to observe the condition of the mother after giving birth, a newborn and children of different ages, and manages the basic skills in nursing implementation
- knows the basics of emergency childbirth and is able to act appropriately in the case of resuscitation
- masters the principles of infection control and acts aseptically
- is able to safely carry out pharmacotherapy and fluid therapy for children of different ages and the mother, under supervision
- takes into account the health promotion perspective in the care of children of different ages and the whole family
- is able to manage the guidance of children of different ages and the whole family and is able to use different guidance methods
- promotes patient safety at all phases of the care process
- follows the rules of confidentiality in the privacy policy
- follows the rules of supervised practical training in his/her own actions
- acts responsibly by recognising the limits of his/her expertise and actively seeks information
- is able to reflect on his/her own actions in a realistic way and is able to give and receive feedback

Fail
The student
- lacks of understanding of professional entercounters: treats patients of different ages and the whole family, as well as members of the work community, in an inappropriate and disrespectful way and is not able to interact with them professionally
- does not perceive the care path for the woman giving birth, children of different ages and the whole family
- lacks basic knowledge and skills in the substance domain; seeking and utilising knowledge is inadequate and there are misunderstandings of theory and skills even after the steering discussions;
- is not able to plan, implement and assess client-centered care for children of different ages and for the whole family
- is not able to observe the health of the woman who has given birth, the newborn and children of different ages
- does not know the basics of emergency delivery and resuscitation
- cannot demonstrate the essential clinical skills, even under supervision
- does not know the principles of infection control or aseptic technique
- makes mistakes in the patient's pharmacotherapy and fluid therapy despite being guided, has not completed his/her medication pass
- shows no interest in guiding patients of different ages or their family
- breaks confidentiality or patient privacy
- does not recognise the boundaries of his/her expertise, thereby endangering patient safety
- disregards the agreed rules of the supervised practice (e.g., does not take responsibility for his/her own learning; doesn’t reach his/her goals, does not follow the working hours, does not report his/her absences, the amount of the working hours are lacking).
- assesses his/her own competence unrealistically and is not able to receive feedback

Assessment methods and criteria

Osallistuminen ohjattuun harjoitteluun määrätyn tuntimäärän mukaisesti, harjoittelun tavoitteiden, työaikasuunnitelman, arvioinnin ja kirjallisen tehtävän tekeminen ohjeiden mukaan. Lääkehoidon passin täyttäminen. Ohjattu harjoittelu arvioidaan hyväksytty/hylätty. Arviointiin sisältyy opiskelijan itsearviointi, ohjaajan arviointi ja ohjaavan opettajan arviointi (sisältää kirjallisten tehtävien arvioinnin).

Teaching methods

Osallistuminen ohjattuun harjoitteluun määrätyn tuntimäärän mukaisesti. Pakollinen läsnäolovelvollisuus harjoittelun orientaatiossa. Harjoittelun tavoitteiden, itsearviointien sekä kirjallisten tehtävien tekeminen ohjeiden mukaan. Lääkehoidon passin täyttäminen.

Student workload

Laajuus 6op eli 6 x 27h = 162h : 2 = 81h/harjoitteluosasto. Harjoittelussa tehdään osastokohtaisesti vähintään 76,5h/2vk (38h 15min/vk), jolloin harjoitteluosastoilla tehtävä yhteistuntimäärä on 153h. Kokonaistuntimäärä sisältää arvioinnit osastolla.

Harjoittelun orientaatiossa läsnäolovelvollisuus. Mahdolliset loppureflektiot koululla opettajan kanssa sopimuksen mukaan.

Practical training and working life cooperation

Harjoittelu ajoittuu syksylle 2022 tutoropettajan tekemän ryhmäjaon mukaisesti tai oman erillisen ohjeen mukaan. Kaksi viikkoa synnyttäneiden osastolla ja kaksi viikkoa lasten osastolla, yhteensä 4 viikon yhtenäinen jakso.

Opiskelija kirjaa kummastakin harjoittelupaikasta erikseen alustavat tavoitteet arviointilomakkeelle, keskustelee tavoitteistaan ohjaajan kanssa ja lähettää ne sähköpostitse opettajalle ja kopiona ohjaajilleen ensimmäisen harjoitteluviikon torstaina. Työvuorot suunnitellaan mahdollisuuksien mukaan ohjaajan työvuorojen mukaan. Ne kirjataan työvuorolomakkeelle ja lähetetään opettajalle ja ohjaajalle yhdessä tavoitteiden kanssa.
Opiskelija täyttää harjoittelujaksolla sekä lääkehoitopassia.

- substanssialueen perustiedoissa ja taidoissa on selviä puutteita, tietojen etsiminen ja hyödyntäminen on puutteellista ja väärinymmärryksiä teoriatiedoissa ja taidoissa vielä ohjauskeskusteluiden jälkeenkin
- ei hahmota potilaan hoitopolkua
- ei osaa keskeisiä kliinisisiä taitoja ohjattunakaan
- ei hallitse infektioiden torjunnan periaatteita ja eikä aseptista toimintaa
- tekee virheitä potilaan lääke- ja nestehoidossa ohjauksesta huolimatta, ei ole täyttänyt lääkehoidon passia
- ei osaa tarkkailla potilaan peruselintoimintoja ja vointia eikä hallitse niihin liittyvää päätöksentekoa
